A derma roller is a handheld microneedling tool. The FDA describes manual rollers as creating repeated punctures as they move over skin. It says it has authorised a limited number of microneedling devices for specific uses and body areas. That is US guidance.

Needle length depends on the procedure, and rollers are made in lengths from under 0.2 mm up to 3 mm. Singh and Yadav describe home-care rollers well under 0.5 mm, including lengths below 0.15 mm and about 0.2 mm. They describe standard medical rollers as 0.5 to 3 mm. Their 1.5 to 2 mm range for scar work refers to clinics, not a home-use recommendation. This roller is 0.5 mm. If you are unsure whether that length suits you, ask a GP, pharmacist or dermatologist.

Do not use a roller over broken, inflamed or infected skin. The NHS advises people with acne not to pick or squeeze spots or scrub their skin, as this can irritate it and worsen acne. For its clinical microneedling service, Chelsea and Westminster Hospital NHS Foundation Trust lists active skin infection, pregnancy and a history of keloid scarring among the precautions. Ask a GP or dermatologist before use if any of these apply to you.

Speak with a GP or dermatologist first if you have eczema or psoriasis, a history of raised scarring or cold sores, or a recent skin treatment. Singh and Yadav list several skin conditions and infection risks in a clinical review, but that is not a home-device suitability checklist. The NHS advises telling healthcare professionals about anticoagulants before an invasive procedure, because a cut can cause heavy bleeding. Ask your prescriber or pharmacist, and do not stop prescribed medicine yourself.

Microneedling can cause bleeding, discomfort, irritation and infection. For its clinical procedure, Chelsea and Westminster Hospital NHS Foundation Trust lists warmth, redness, peeling, pinpoint bleeding and possible pigment changes. The FDA also lists possible infection, skin damage and skin reactions. These are risks described for microneedling, not rates for this roller.

Wash your hands and start with clean skin. Follow the box instructions for technique, hygiene and replacement. Iriarte and colleagues describe granulomatous reactions linked to topical products not approved for intradermal use. Do not put products through freshly needled skin unless a qualified professional advises you to.
